SUBJECT: STANDARDIZED CREDENTIALING APPLICATION FORM TO BE USED BY ALL LICENSED COMPANIES IN KANSAS FOR CREDENTIALING HEALTHCARE PROVIDERS.

The Kansas Insurance Department (KID) has approved a Standardized Credentialing Application Form (SCAF) to be used by all Kansas licensed Health Care Organizations that credential healthcare providers. The states of Colorado, Nebraska and Missouri have no objections to this form being used in their states.

The use of this form is voluntary, however, it will become mandatory in 2001 through rule or regulation, and the KID strongly suggests that this form be used starting now. In an effort to simplify credentialing for those who do business in both Kansas and Missouri, the KID and the Missouri Department of Insurance have agreed to accept one another's form.

Companies must add their own attestation clause and release form, and may further ask for anything additional, but may not remove any of the information requested on the SCAF. For recredentialing, companies should request only the information on the SCAF that needs to be updated. Health Care Organizations are encouraged to read the SCAF carefully to avoid asking for additional information already provided in the form.

The SCAF will be available at the KID internet homepage http://www.ink.org/public/kid/ in pdf format. The website will also contain information about software options that may be available to automate the completion of the SCAF.
